To prepare for your first visit at Premium Hearing Care, please complete the patient  forms below and bring them to your appointment. Please download the patient intake form, then print it out to complete it and bring it to your appointment. You’re also welcome to complete this form in-office by simply arriving 15 minutes before your scheduled appointment time. You can also download and review the HIPAA form, but you will sign this form in our office.
